A maintenance card is available for download under "Your Certifications" in "Your Account."

An official printed maintenance card is available for a fee of $35 which includes FedEx delivery.  This can ONLY be delivered to a street address - NO PO Boxes or military addresses.

Certificates - Certification, Certificate of Added Qualification and Self Assessment 

Need a duplicate certificate for your certification or certificate of added qualification or for a Self Assessment module you have completed?  There is a $10 fee.

For those seeking a replacement for online self assessment modules, please check your account (see link at the top of the page) to see if you can directly download it from there before you place a replacement order.
